Estimates major-bleeding risk during extended (stable-phase) anticoagulation in patients treated for venous thromboembolism, stratifying into lower- and elevated-risk groups to inform prolonged-treatment decisions; for the stable phase, not the initial acute treatment period.

Anemia (Hgb < 13 g/dL men, < 12 g/dL women) — +1.5

History of bleeding (prior major/CRNM bleed, rectal/nose bleeding, or hematuria) — +1.5

Age ≥ 60 years — +1.5

Renal dysfunction (eGFR < 60 mL/min by Cockcroft-Gault; CrCl 30–60) — +1.5

Active cancer — +2

Uncontrolled arterial hypertension (SBP ≥ 140 mmHg) — +1 in males only
