TASH Score (Trauma Associated Severe Hemorrhage)

Estimates the probability of massive transfusion after major blunt or multiple trauma from vitals, hemoglobin, base excess, FAST, sex, and fracture pattern to help decide whether to activate a massive transfusion protocol; it predicts transfusion need, not whether to give blood, and never overrides bedside judgment.

Heart rate (bpm; >120 = +2, otherwise 0)

Hemoglobin (g/dL; <7 = +8, <9 = +6, <10 = +4, <11 = +3, <12 = +2, ≥12 = 0)

Base excess (arterial mmol/L; enter negative for a deficit; <−10 = +4, <−6 = +3, <−2 = +1, ≥−2 = 0)

Positive FAST (free intra-abdominal fluid)

Clinically unstable pelvic fracture

Open or dislocated femur fracture
