Lab-free primary-care decision rule scoring five history and exam findings to estimate whether chest pain is from coronary heart disease; a high negative-predictive-value rule-out aid, not a substitute for ECG, troponin, or ACS workup.

Sex (selects the age cut-off for item 1: female ≥ 65, male ≥ 55; sex alone scores no point)

Known vascular disease (coronary, cerebrovascular, or peripheral arterial) (+1)

Pain worse with exercise / exertion (+1)

Pain reproducible by palpation of the chest wall (REVERSED: pain that is NOT reproducible scores +1)

Patient assumes the pain is of cardiac origin (+1)
