Hematopoietic Cell Transplantation Comorbidity Index (HCT-CI)

Pre-transplant comorbidity index that sums weighted organ-system findings to stratify non-relapse mortality risk before allogeneic hematopoietic cell transplantation, with an optional age adjustment.

Arrhythmia (AF/flutter, sick sinus syndrome, or ventricular arrhythmia)

Cardiac (CAD, CHF, MI, or ejection fraction ≤50%)

Inflammatory bowel disease (Crohn disease or ulcerative colitis)

Diabetes (insulin or oral hypoglycemics; diet-controlled = No)

Cerebrovascular disease (TIA or CVA)

Psychiatric disturbance (depression/anxiety requiring treatment)

Obesity (BMI >35 kg/m²; exactly 35 = No)

Infection (antimicrobials continued after day 0)

Rheumatologic: SLE, RA, polymyositis, MCTD, or PMR (weight 2)

Peptic ulcer requiring treatment (weight 2)

Moderate/severe renal: creatinine >2 mg/dL (strict), dialysis, or prior transplant (weight 2)

Prior solid tumor at any time, excluding nonmelanoma skin cancer (weight 3)

Heart valve disease, except mitral valve prolapse (weight 3)

Hepatic disease (single organ; highest applicable tier)

Pulmonary disease (single organ; highest applicable tier)

Add age point (HCT-CI/Age, +1 if age ≥40 years)

Age (years; only used when age adjustment = Yes)
