Quick five-item frailty screen in older adults (fatigue, resistance, ambulation, illness burden, and weight loss) that sorts people into robust, pre-frail, or frail to flag those at higher risk of disability and decline.

Fatigue: felt tired all or most of the time during the past 4 weeks

Resistance: any difficulty walking up 10 steps without resting, on your own and without aids

Ambulation: any difficulty walking several hundred yards, on your own and without aids

Illnesses: number of these 11 chronic conditions a doctor has diagnosed (hypertension, diabetes, cancer [excl. minor skin], chronic lung disease, MI, CHF, angina, asthma, arthritis, stroke, kidney disease)
